Transfusions add risk in some heart attacks, finds study of patients with anemia

Dec. 24, 2012 ? When heart attack patients present in the emergency department with some degree of anemia, or anemic patients have a heart attack, physicians have a tendency, but not much guidance, about whether to provide a blood transfusion. The idea is that a transfusion could help more oxygen get to the heart. Recent national guidelines suggested that there simply isn't good evidence to encourage or discourage the common practice, but a new meta-analysis of 10 studies involving more than 203,000 such patients comes down on the side of it increasing the risk of death.

The next step for determining when the practice could be appropriate needs rigorous randomized trials that will generate more decisive, high-quality data, said lead author Dr. Saurav Chatterjee, a cardiology fellow at the Alpert Medical School of Brown University and the Providence VA Medical Center.

For the analysis published Dec. 24 in the Archives of Internal Medicine, Chatterjee and his co-authors combined and analyzed data from studies in which anemia patients with heart attacks either received "liberal" transfusions or received more restricted versions of the treatment or no transfusions at all. Liberal transfusions were defined as cases in which patients either received two units of blood or more or had a transfusion even with a hematocrit reading (a measure of red blood cell concentration) higher than 30 percent (normal is in the low 40s).

What the researchers found, after statistical adjustments to control for important medical factors, was that the risk of death was 12 percent higher for people who received the liberal transfusions than those who did not. Moreover, the group that received liberal transfusions had twice the odds of having another heart attack.

"What we found is that the possibility of real harm exists with transfusion," Chatterjee said. "It is practiced in emergency departments all across the United States. I think it is high time that we need to answer the question definitively with a randomized trial."

Of the 10 papers that Chatterjee and his co-authors reviewed, all but one were observational studies. The only randomized trial was a small pilot experiment.

Searching for an answer

Chatterjee began the study when he was a resident at Maimonides Medical Center in New York. He noticed a paper by the AABB (formerly the American Association of Blood Banks) in which the association said there was not enough clinical evidence for or against transfusions in heart attack patients.

For clinicians, the practice has always been a tough judgment call. Some transfusions are clearly necessary, for example when a patient's troubles include not just a heart attack but also severe ongoing bleeding, Chatterjee said. But transfusions also create health risks, such as an increase in potential clotting because platelets may clump together more, or from an inflammatory immune response to the introduction of blood of a "foreign" source into the body.

Chatterjee and his co-authors decided to comb the literature to determine whether, if properly combined and analyzed, existing data could provide some insight. They found 729 potentially relevant studies, but only 10 that had the right data to help answer the question.

Few as they were, Chatterjee said, the studies all told much the same story.

"One of the things that struck us is that there were very few studies in evidence of transfusion at all," Chatterjee said. "In our case, though, we found that the effect was pretty consistently harmful across the spectrum of studies, spectrum of time, and spectrum of patients that were enrolled in the individual studies."

Chatterjee said the study should not be taken to mean that transfusions should be stopped altogether for anemic heart attack patients. Instead, he said, doctors must continue exercising their clinical judgment, at least until results from a large, well-designed randomized trial can be produced. Mindful of the risk his study found, however, they might just want to shift their thinking about where the border is among borderline cases.

"Before a definitive trial is out there, we should be conservative, especially considering the high risk of harm," he said.

In addition to Chatterjee, the paper's other authors are Jorn Wetterslev of the Centre for Clinical Intervention Research in Copenhagen, Denmark; Abhishek Sharma and Edgar Lichstein of Maimonedes Medical Center; and Debabrata Mukherjee of Texas Tech University.

(AP) ? U.S. consumers peering over the "fiscal cliff" don't like what they see.

Fears of sharp tax increases and government spending cuts set to take effect next week sent consumer confidence tumbling in December to its lowest level since August.

The Conference Board said Thursday that its consumer confidence index fell for the second straight month in December to 65.1, down from 71.5 in November.

The survey showed consumers' outlook for the next six months deteriorated to its lowest level since 2011 ? a signal to Lynn Franco, the board's director of economic indicators, that consumers are worried about the tax hikes and spending cuts that take effect Jan. 1 if the White House and Congress can't reach a budget deal.

Earlier this week a report showed consumers held back shopping this holiday season, another indication of their concerns about possible tax increases.

The December drop in confidence "is obvious confirmation that a sudden and serious deterioration in hopes for the future took place in December ? presumably reflecting concern about imminent 'fiscal cliff' tax increases," said Pierre Ellis, an economist with Decision Economics.

The decline in confidence comes at a critical time when the economy is showing signs of improvement elsewhere.

A recovery in housing market is looking more sustainable. On Thursday, the government said new-home sales increased in November at the fastest seasonally adjusted annual pace in 2? years.

And the job market has made slow but steady gains in recent months. The average number of Americans applying for unemployment benefits over the past month fell to the lowest level since March 2008.

But the political wrangling in Washington threatens the economy's slow, steady progress. President Barack Obama and House returned to Washington Thursday to resume talks with just days to go before the deadline.

Mixed signals over those negotiations led to a rocky day on Wall Street.

Stocks plunged early after the weak consumer confidence report and a warning from Senate Majority Leader Harry Reid that the government appeared to be headed over the "fiscal cliff." At one point, the Dow Jones industrial average fell as much as 150 points.

But the market came back in the final hour of trading on a potential sign of movement in the talks: Republican leaders announced they would bring the House back into session on Sunday evening. The Dow recouped nearly all of its losses to close down just 18 points at 13,096.

A short fall over the cliff won't push the economy into recession. But most economists expect some tax increases to take effect next year. That could slow economic growth.

While consumers are more worried about where the economy is headed, they were upbeat about present conditions, according to the latest survey. Their assessment of current economic conditions rose this month to the highest level since August 2008.

A key reason for that is gas prices hit a 2012 low of $3.21 a gallon last week. Normally, that would prompt consumers to spend more on holiday shopping.

But the opposite has happened. A report from MasterCard Advisors Spending pulse indicated sales grew in the two months before Christmas at the weakest rate since 2008, when the country was in a deep recession.

There were other distractions this holiday season. In late October, Superstorm Sandy battered the Northeast and mid-Atlantic states, which account for 24 percent of U.S. retail sales. That coupled with the presidential election, hurt sales during the first half of November.

Shopping picked up in the second half of November. But "fiscal cliff" worries dampened sales in December.

The National Retail Federation, the nation's largest retail trade group, remains optimistic that sales won't be quite as bad as earlier reports have suggested. It is sticking to its forecast for total sales for November and December to be up 4.1 percent to $586.1 billion this year. That's more than a percentage point lower than the growth in each of the past two years, and the smallest increase since 2009 when sales were up just 0.3 percent.

Direct Sales Business - 5 Reasons Why It Might Be Perfect For You

What is direct sales business? Wikipedia defines it as ?marketing and selling of products directly to consumers away from a fixed retail location?. It also often, but not always, uses multi level marketing or network marketing as a business model.

Direct sales business is not everyone?s cup of tea, that?s for sure. Some people just don?t have the necessary drive to succeed. Like any business, it requires self discipline, belief and hard work to succeed.

1. Be your own boss.

You?ve worked for others long enough. You?re ready to strike out on your own and start your own direct sales or network marketing business. Be sure you have done some research about what owning your own business entails before charging forward with your plan. Know how much your initial fees will be, how many other associates are in your area, and what kind of compensation plan is available with the company you choose.

2. Success breeds success.

Someone you know may be working in a direct sales company in a different town from where you live. She?s very successful at what she does and her enthusiasm is contagious. She invites you to be part of her team, but take your time making a decision.

Before you sign that contract and pay for your start-up kit, do some research to see how successful she really is, as well as doing some checking on her upline. If you find that they have a successful upline that is supportive of her, as well as a satisfied downline, perhaps now is the time to sign on.

3. Belief is important.

You have found a product that you feel is perfect. It does exactly what it was advertised to do, and it is something you can?t see yourself doing without. In fact, you?ve told all of your friends about it because it?s so good. If this describes you, you may have found the perfect direct sales company to work with.

You must love the product your company offers in order to be successful at selling it. This is the first, and most important, decision to make before choosing a direct sales company. If you don?t love, use, and believe in the products you sell, it will be evident to those you approach.

4. Do you love it?

If the direct sales company you are considering is associated with an activity, is that activity something you?re passionate about? Having a passion about scrapbooking, cooking, or whatever the activity, will help your customers see that you?re trying to share something with them rather than trying to ?sell them something?.

5. How flexible are you?

Are your children at an age where they don?t need you quite as much as they used to? If you have some flexibility in your schedule, direct sales might be a viable option. Not only can you choose the days and times that you work, you also have some degree of control over what you earn. If you are serious about growing a business, you are only limited by the amount of time and effort you use to do so.

If you think you have found the direct sales company that you can get behind 100%, congratulations! Take your time learning all you can learn and then step out into your new life. You may find, after doing your research, that you and direct sales are perfect together ? a match made in heaven.

However, in addition to the 5 points made before you should also check whether your company or upline provides training so you can build your business using modern, Internet based strategies. Building your direct sales business online is the most effective way to reach a massive amounts of interested people. It has to be done correctly though and that?s why we provide you training and tools that work with any network marketing and direct sales business. Click here to learn more.

No deal in sight as deadline for fiscal deal nears

WASHINGTON (AP) ? Lawmakers are engaged in a playground game of "who goes first," daring each political party to let the year end without resolving a Jan. 1 confluence of higher taxes and deep spending cuts that could rattle a recovering, but-still-fragile economy.

President Barack Obama returns from Hawaii Thursday to this increasingly familiar deadline showdown in the nation's capital, with even a stopgap solution now in doubt.

Adding to the mix of developments pushing toward a "fiscal cliff," Treasury Secretary Timothy Geithner informed Congress on Wednesday that the government was on track to hit its borrowing limit on Monday and that he would take "extraordinary measures as authorized by law" to postpone a government default.

Still, he added, uncertainty over the outcome of negotiations over taxes and spending made it difficult to determine how much time those measures would buy.

In recent days, Obama's aides have been consulting with Senate Democratic Leader Harry Reid's office, but Republicans have not been part of the discussions, suggesting much still needs to be done if a deal, even a small one, were to be struck and passed through Congress by Monday.

At stake are current tax rates that expire on Dec. 31 and revert to the higher rates in place during the administration of President Bill Clinton. All in all, that means $536 billion in tax increases that would touching nearly all Americans. Moreover, the military and other federal departments would have to cut $110 billion in spending.

But while economists have warned about the economic impact of tax hikes and spending cuts of that magnitude, both sides appear to be proceeding as if they have more than just four days left. Indeed, Congress could still act in January in time to retroactively counter the effect on most taxpayers and government agencies, but chances for a large deficit reduction package would likely be put off.

House Republican leaders on Wednesday said they remain ready to negotiate, but urged the Senate to consider or amend a House-passed bill that extends all existing tax rates. In a statement, the leaders said the House would consider whatever the Senate passed. "But the Senate first must act," they said.

Aides said any decision to bring House members back to Washington would be driven by what the Senate does.

Reid's office responded shortly after, insisting that the House act on Senate legislation passed in July that would raise tax rates only on incomes above $200,000 for individuals and $250,000 for couples.

Meanwhile, Obama has been pushing for a variant of that Senate bill that would include an extension of jobless aid and some surgical spending reductions to prevent the steeper and broader spending cuts from kicking in.

For the Senate to act, it would require a commitment from Senate Republican Leader Mitch McConnell not to demand a 60-vote margin to consider the legislation on the Senate floor. McConnell's office says it's too early to make such an assessment because Obama's plan is unclear on whether extended benefits for the unemployed would be paid for with cuts in other programs or on how it would deal with an expiring estate tax, among other issues.

What's more, House Speaker John Boehner would have to let the bill get to the House floor for a vote. Given the calendar, chances of accomplishing that by Dec. 31 were becoming a long shot.

Amid the standoff, Geithner advised Congress on Wednesday that the administration will begin taking action to prevent the government from hitting its borrowing limit. In a letter to congressional leaders, Geithner said accounting measures could save approximately $200 billion.

That could keep the government from reaching the debt limit for about two months. But if Congress and the White House don't agree on how to avoid the "fiscal cliff," he said, the amount of time before the government hits its borrowing limit is more uncertain.

"If left unresolved, the expiring tax provisions and automatic spending cuts, as well as the attendant delays in filing of tax returns, would have the effect of adding some additional time to the duration of the extraordinary measures," he wrote.

Whenever the debt ceiling hits, however, it is likely to set up yet another deadline for one more budget fight between the White House and congressional Republicans.

Initially, clearing the way for a higher debt ceiling was supposed to be part of a large deal aimed at reducing deficits by more than $2 trillion over 10 years with a mix of tax increases and spending cuts, including reductions in health programs like Medicare. But chances for that bargain fizzled last week when conservatives sank Boehner's legislation to only let tax increases affect taxpayers with earnings of $1 million or more.

Obama and his aides have said they would refuse to let Republicans leverage spending cuts in return for raising the debt ceiling. But Republicans say the threat of voting against an increase in the limit is one of the best ways to win deficit reduction measures.

Another potential showdown is pending. A renewed clash over spending could come in late March; spending authority for much of the government expires on March 27.

What wealthy Chinese want for Christmas: a bike

19 hrs.

HONG KONG - Rich Chinese are buying bicycles that cost more than the average citizen makes in three years, motivated by nostalgia for the days when two wheels were the primary means of transport.?

China is now the world's biggest auto market, but high-end bike sales are expected to grow by 10 percent a year as they become a status symbol for wealthy executives.?

Yu Yiqun, the creative director at an advertising company in the Chinese capital, cycles to work on his favorite bike - a 100,000 yuan ($16,000) hand-made Alex Moulton.?

"It might be the only one in Beijing. It's like the Rolls-Royce of bicycles. Very classical, purely hand-made," said the 40-year-old Yu, who has about 35 high-end bikes.?

"I remember my father used to ride me to the city in the winter - about 40 km and minus 30 degrees centigrade. Back then, it was a means of transport that fulfilled your dream of travelling afar, which was relatively cheap but required brawn."?

Yu symbolizes a new bike culture in China, where wealthy, health-conscious executives are upgrading their lifestyle, in some cases abandoning flashy cars and taking to the road on high-end bicycles that can cost more than a car.?

"Demand for mainstream luxury items such as premium cars, watches has come to a point of saturation. High-income groups now turn to high-end bikes to show off the uniqueness in taste and healthy lifestyle," said Zhou Jiannong, general manager of Rbike Networks Ltd in China.?

Analysts estimate about 10 percent annual growth in the Chinese bicycle market over the next few years, with the high-end segment forecast to grow by as much as 15 percent a year.?

Companies are also getting in on the act, with a Hong Kong-based supplier taking an order for 1,000 pricey bikes from a Chinese financial firm as a year-end bonus for employees.?

"People are sick of conventional gifts such as wines and tobacco. For mainlanders, a bike is a great gift that shows your unique lifestyle," said Adam Wong, managing director at Hong Kong's Komda Bicycles.?

Wong declined to name the bank that had ordered the bikes, but he said they had an average price tag of 3,000 yuan ($480).?

Fashion statement
Fashion label Shanghai Tang, eager for a slice of this growing pie, teamed up with Dutch bike maker Colossi Cycling to make bicycles aimed specifically at China, where bike demand is estimated at about 28 million units a year?

"The high-end sector is going to be the major source of growth in the Chinese market. In China, bikes are more than just a means of transportation. It has become a fashion," said Terry Liu, an analyst at Fubon Research in Taiwan.?

It can cost up to HK$300,000 ($38,700) for an imported limited edition of expensive brands such as Italy's Colnago or France's Look, nearly 100 times the price of a Flying Pigeon, China's official bike since it was born in 1950.?

But the cost as no object for many high-income Chinese looking for the best two-wheeled vehicle.?

"For businessmen, they are not looking at the price. They are looking at the quality. They assemble their bike with import components in accordance to their taste and needs," said Zhang Lei, a director of a Zhuhai paper products supplier, who plans to spend 10,000 yuan to upgrade his current bike.?

Yu, the advertising executive in Beijing, has orders in for four more hand-made bikes, expanding his vast collection which includes brands such as Trek, Bianchi and Colnago.?

He and his wife have two cars but he says he doesn't drive.?

"I always bring my bike when I go on a business trip," Yu said. "When I go to Harbin, I bring a small, folding bicycle since it's easier for me to get around the city. When I go to Dalian, I bring a bigger bike since it's a mountainous city."?

ROME (AP) ? Italy's caretaker Premier Mario Monti said Sunday he won't run in February elections, but if political parties that back his anti-crisis agenda ask him to head the next government he would consider the offer.

Monti ruled out heading any ticket himself, saying "I have no sympathy for 'personal' parties."

At a news conference, Monti made clear he was spurning an offer from his predecessor Silvio Berlusconi to run on a center-right election ticket backed by the media mogul, citing Berlusconi's heavy criticism of his economic policies.

Monti's decision ends weeks of speculation that have dominated Italian politics and preoccupied Europe, which is eager to see Monti's financial reforms continue.

The premier, an economist who has spent 13 months tasked with trying to right Italy's troubled economy, said Berlusconi's flipping back and forth between condemning the government's economic policies and then praising the premier convinced him that "I couldn't accept his offer."

Monti was tapped by Italy's president to lead the country after Berlusconi was forced to resign, having lost the confidence of international markets. He stepped down Friday after Berlusconi's party withdrew its support from his technical government, but has been asked stay on in a caretaker capacity in the run-up to Feb. 24-25 elections.

Other centrist parties in Parliament have been urging him to run for another stint as premier. Monti said "I won't line up with anyone," but made clear he was available to head the next government.

"If one or more political forces is credibly backing (Monti's) agenda or even has a better one, I'd evaluate the offer," Monti said.

"To those forces who demonstrate convincing and credible adherence to (my) agenda, I will be ready to give encouragement, and if necessary, lead" the country, he said.

Monti expressed gratitude to Berlusconi for his backing of key anti-crisis measures, but said "I struggle to understand his line of thought."

"Yesterday, we read that he assessed the work of the (Monti) government to be a complete disaster. A few days earlier I read flattering things," he said.

The logic of Berlusconi's positions "escapes me" and "I couldn't accept his offer," Monti said, drawing chuckles.

Monti praised his government and Parliament for its support of spending cuts, new taxes and pension reform that he said had saved Italy from succumbing to the debt crisis.

"Italians as citizens can hold their heads up high in Europe," Monti said, noting Italy had avoided the kind of bailouts that Greece, Portugal, Ireland and Cyprus have been forced to take.

Italy's President Giorgio Napolitano dissolved Parliament after Monti handed in his resignation following approval of the country's national budget law. Monti noted that as a senator-for-life, he remains in Parliament and thus doesn't need to run for a seat in the legislature.

Napolitano set elections two months before their expected date, recognizing that without Berlusconi's support for Monti, it was useless to wait until late April for Italians to go to the ballot box.

Voter opinion polls indicate a centrist ticket backing Monti would take about 15 percent of the vote, meaning any government headed by him would have to have the support of either of Italy's largest political groupings: the center-right, led by Berlusconi, or the center-left, led by Pier Luigi Bersani.

sultry roil: Do It Yourself Home Improvement ? Essential Factors ...

Preparing for a DIY home improvement task can be quite thrilling. Whether you?re painting your house, installing new flooring or building a deck, you can look forward to how great your home will look in a few months. Although, before you initiate the process, you need to be sure that you have planned everything right, in order for things to turn out the way you would like them to.

Before you begin any form of DIY home improvement task, make sure that it is sensible and that you can fit it into your schedule and that you have the right skills to get it done. The idea of taking on a large project is something many homeowners are zealous about and then they either lose their oomph amidst it or learn that it was much trickier than they had imagined. Some tasks take quite a bit more time and expertise than others and you should think about this sincerely before you begin work. If needed, look for some guidance form an experienced individual who knows about this type of task. It is more excellent to not even start a project or hire someone else than to start it, than to never finish.

We want to say a fast word about our conversation re home improvement. Take a look at what is occurring on your end, and that may help you to refine what you need. There are always some points that will have more of an influence than others. No matter what, your careful attention to the matter at hand is one thing you and all of us have to do. The latter half of our talk will center on a few highly relevant issues as they concern your possible circumstances.

One way to make a big difference in how your rooms look is by putting in new carpets. One of the improvement jobs you might want to do on your own is laying your new carpet. Having carpets installed professionally is quite expensive, and it can save you quite a bit to do it yourself. More expensive carpet is way nicer, so saving on labor will allow you to put the money into better quality. There are many styles and varieties of carpet, and you should make your selection carefully. Choosing the carpet and its color will depend on how well it matches the furniture and walls of the rest of your house. It will take you some time and work to install your own carpet, and the right tools will have to be gotten some where.

Saving money is the most important reason for doing home improvement jobs by yourself. Don?t reduce the quality of tools or materials, just to save money. There?s nothing wrong with buying quality used or recycled items, but don?t buy cheap materials that are going to affect the results of your project. Choosing the right interior or exterior paint, is a good example of this. Cheaper paint will cost you more money in the long run, because you will have to repaint sooner, and it won?t look as good either. In most cases, inexpensive materials fall apart quicker, so items like cheap tile which will break in a year or two, should not be purchased.

DIY home improvement projects involve many factors, which we have looked at. Any more the best place to look for information might be online, but there are plenty of books and magazines, as well as TV programs with plenty of information. With the right planning, you can make many repairs, add an addition to your home or remodel a room. Your home can be changed to look better with lots of possibilities.

Over the fiscal cliff: Soft landing or dizzy dive?

WASHINGTON (AP) ? Efforts to save the nation from going over a year-end "fiscal cliff" were in disarray as lawmakers fled the Capitol for their Christmas break. "God only knows" how a deal can be reached now, House Speaker John Boehner declared.

President Barack Obama, on his way out of town himself, insisted a bargain could still be struck before Dec. 31. "Call me a hopeless optimist," he said.

A look at why it's so hard for Republicans and Democrats to compromise on urgent matters of taxes and spending, and what happens if they fail to meet their deadline:

___

NEW YEAR'S HEADACHE

Partly by fate, partly by design, some scary fiscal forces come together at the start of 2013 unless Congress and Obama act to stop them. They include:

? Some $536 billion in tax increases, touching nearly all Americans, because various federal tax cuts and breaks expire at year's end.

? About $110 billion in spending cuts divided equally between the military and most other federal departments. That's about 8 percent of their annual budgets, 9 percent for the Pentagon.

Hitting the national economy with that double whammy of tax increases and spending cuts is what's called going over the "fiscal cliff." If allowed to unfold over 2013, it would lead to recession, a big jump in unemployment and financial market turmoil, economists predict.

___

WHAT IF THEY MISS THE DEADLINE?

If New Year's Day arrives without a deal, the nation shouldn't plunge onto the shoals of recession immediately. There still might be time to engineer a soft landing.

So long as lawmakers and the president appear to be working toward agreement, the tax hikes and spending cuts could mostly be held at bay for a few weeks. Then they could be retroactively repealed once a deal was reached.

The big wild card is the stock market and the nation's financial confidence: Would traders start to panic if Washington appeared unable to reach accord? Would worried consumers and businesses sharply reduce their spending? In what could be a preview, stock prices around the world dropped Friday after House Republican leaders' plan for addressing the fiscal cliff collapsed.

Federal Reserve Chairman Ben Bernanke has warned lawmakers that the economy is already suffering from the uncertainty and they shouldn't risk making it worse by blowing past their deadline.

___

WHAT IF THEY NEVER AGREE?

If negotiations between Obama and Congress collapse completely, 2013 looks like a rocky year.

Taxes would jump $2,400 on average for families with incomes of $50,000 to $75,000, according to a study by the non-partisan Tax Policy Center. Because consumers would get less of their paychecks to spend, businesses and jobs would suffer.

At the same time, Americans would feel cuts in government services; some federal workers would be furloughed or laid off, and companies would lose government business. The nation would lose up to 3.4 million jobs, the Congressional Budget Office predicts.

"The consequences of that would be felt by everybody," Bernanke says.

___

THE TAXES

Much of the disagreement surrounds the George W. Bush-era income tax cuts, and whether those rates should be allowed to rise for the nation's wealthiest taxpayers. Both political parties say they want to protect the middle-class from tax increases.

Several tax breaks begun in 2009 to stimulate the economy by aiding low- and middle-income families are also set to expire Jan. 1. The alternative minimum tax would expand to catch 28 million more taxpayers, with an average increase of $3,700 a year. Taxes on investments would rise, too. More deaths would be covered by the federal estate tax, and the rate climbs from 35 percent to 55 percent. Some corporate tax breaks would end.

The temporary Social Security payroll tax cut also is due to expire. That tax break for most Americans seems likely to end even if a fiscal cliff deal is reached, now that Obama has backed down from his call to prolong it as an economic stimulus.

___

THE SPENDING

If the nation goes over the fiscal cliff, budget cuts of 8 or 9 percent would hit most of the federal government, touching all sorts of things from agriculture to law enforcement and the military to weather forecasting. A few areas, such as Social Security benefits, Veterans Affairs and some programs for the poor, are exempt.

___

THERE'S MORE AT STAKE

All sorts of stuff could get wrapped up in the fiscal cliff deal-making. A sampling:

? Some 2 million jobless Americans may lose their federal unemployment aid. Obama wants to continue the benefits extension as part of the deal; Republicans say it's too costly.

? Social Security recipients might see their checks grow more slowly. As part of a possible deal, Obama and Republican leaders want to change the way cost-of-living adjustments are calculated, which would mean smaller checks over the years for retirees who get Social Security, veterans' benefits or government pensions.

? The price of milk could double. If Congress doesn't provide a fix for expiring dairy price supports before Jan. 1, milk-drinking families could feel the pinch. One scenario is to attach a farm bill extension to the fiscal cliff legislation ? if a compromise is reached in time.

? Millions of taxpayers who want to file their 2012 returns before mid-March will be held up while they wait to see if Congress comes through with a deal to stop the alternative minimum tax from hitting more people.

___

CALL THE WHOLE THING OFF?

In theory, Congress and Obama could just say no to the fiscal cliff, by extending all the tax cuts and overturning the automatic spending reductions in current law. But both Republicans and Democrats agree it's time to take steps to put the nation on a path away from a future of crippling debt.

Indeed, the automatic spending cuts set for January were created as a last-ditch effort to force Congress to deal with the debt problem.

If Washington bypassed the fiscal cliff, the next crisis would be just around the corner, in late February or early March, when the government reaches a $16.4 trillion ceiling on the amount of money it can borrow.

Boehner says Republicans won't go along with raising the limit on government borrowing unless the increase is matched by spending cuts to help attack the long-term debt problem. Failing to raise the debt ceiling could lead to a first-ever U.S. default that would roil the financial markets and shake worldwide confidence in the United States.

To avoid that scenario, Obama and Boehner are trying to wrap a debt limit agreement into the fiscal cliff negotiations.

___

SO WHAT'S THE HOLDUP?

They're at loggerheads over some big questions.

Obama says any deal must include higher taxes for the wealthiest Americans. Many House Republicans oppose raising anyone's tax rates. Boehner tried to get the House to vote for higher taxes only on incomes above $1 million but dropped the effort when it became clear he didn't have the votes.

Republicans also insist on deeper spending cuts than Democrats want to make. And they want to bring the nation's long-term debt under control by significantly curtailing the growth of Medicare, Medicaid and Social Security ? changes that many Democrats oppose.

Obama, meanwhile, wants more temporary economic "stimulus" spending to help speed up a sluggish recovery. Republicans say the nation can't afford it.

___

IT'S NOT JUST WASHINGTON

Seems like they could just make nice, shake hands and split their differences, right?

But there's a reason neither side wants to give ground. The two parties represent a divided and inconsistent America. True, Obama just won re-election. But voters also chose a Republican majority in the House.

Republican and Democrats alike say they are doing what the voters back home want.

Neither side has a clear advantage in public opinion. In an Associated Press-GfK poll, 43 percent said they trust the Democrats more to manage the federal budget deficit and 40 percent preferred the Republicans. There's a similar split on who's more trusted with taxes.

About half of Americans support higher taxes for the wealthy, the poll says, and about 10 percent want tax increases all around. Still, almost half say cutting government services, not raising taxes, should be the main focus of lawmakers as they try to balance the budget.

When asked about specific budget cuts being discussed in Washington, few Americans express support for them.

___

THE COUNTDOWN

Time for deal-making is short, thanks to the holiday and congressional calendars. Some key dates for averting the fiscal cliff:

? Lawmakers aren't expected to return to the Capitol until after Christmas, leaving less than a week to vote on a compromise before year's end.

? Obama and his family also were leaving town for a Christmas vacation in Hawaii. The president said because the fiscal cliff was still unresolved, he would return to Washington next week.

? If lawmakers reach Dec. 31 without a deal, some economists worry that the financial markets might swoon.

? The current Congress is in session only through noon Eastern time on Jan. 3. After that, a newly elected Congress with 13 new senators and 82 new House members would inherit the problem.

6 Social Media Changes That Will Rattle Business | MyVenturePad

LikeSocial media has already had a huge impact on businesses both large and small. But according to the latest projections from the research firm International Data Corp. (IDC), there?s a lot more change coming. IDC analysts expect the coming year to be pivotal for social media in business with (among other things) a new emphasis on using social to grow profits, not just gather followers.

New social-friendly devices and computer operating systems will spur growth of so-called ?social business? ? business conducted via social media. New types of ?discovery? shopping will evolve from a combination of tablet usage and an emphasis on visual, rather than text-based messages.

Drawing from the latest IDC research and internal brainstorming sessions among IDC?s analysts, here are six key social business and social media changes that could have a major impact on how you do business in the year ahead (follow us @140Main or check BizBest?s Social Media section for future updates):

1. Social platforms become the new shop fronts

Until now, most businesses have used social media as an extension of their public relations or marketing efforts, focusing attention on such things as news and updates. But as more and more small companies expand sales and customer service efforts to multiple channels, social media will become a true social business platform. In essence, social media will spawn digital storefronts for everything from local businesses to multinational corporations.

2. Sales supersede ?likes? and followers

As business owners and marketers in general continue gaining experience with Facebook and other social platforms, they will become more demanding about the return they get on their social media investments. With businesses questioning the actual value of ?likes?, fans and followers, attention will shift to real results and outcomes of social media efforts. It will become increasingly important to measure and track results from social media, and apply the information to your own definition of success.

3. Visual social takes off

Online marketing efforts that include a visual component (images, video, info graphics, etc.) get better results than text-only. As this sinks in, social platforms (Facebook, Google+, Twitter, LinkedIn) will add new features and focus on visual networking. ?The success of the image-sharing site Pinterest is just one example of how popular and effective visuals have become in the digital world. The increasing popularity of tablets such as iPads ? which are great for visuals ? help drive this change. The newspaper industry currently has a major joint effort underway to reinvent the standard circular into a whole new digital discovery experience for tablet (visit Wanderful.com for info).

4. Groupon?s last gasp

As Groupon continues to struggle and face competition from countless similar services, the Groupon approach will give way to new types of group purchasing conducted via social media communities and what?s being called ?Social Local Mobile.? In SoLoMo, local businesses make offers directly to customers on their mobile devices.

5. Social business gets a boost from new devices and operating systems

Apple and Microsoft are both integrating social into the newest versions of their mobile and desktop operating systems. As a result, social media will become an easier experience for the user and more effective for businesses that understand and leverage the new social business environment. With the release of each new device and system, the social media experience becomes a more integral part of everyday life for customers and prospects.

6. Gap between ?haves? and ?have-nots? widens

Despite the massive growth of social media, business owners and executives remain polarized on the role social media should play in running a business. The pro-social camp will continue to explore social not just as something new and shiny, but also as the very foundation of their businesses. They will expand their efforts on multiple fronts. The social doubters, on the other hand, see Facebook?s 2012 stock offering debacle as confirmation that the value of social media remains in question. This will give the social media adopters a continued advantage, just as businesses that embraced the Internet early on gained an edge.

Social networking has become a global rage. From individuals to big companies ? everyone?s a part of the social networking circuit, whether it is for personal interaction or business growth. Not only does social networking create awareness in terms of business, but it also attracts the appropriate customers. Through various social networking sites like Facebook and Twitter, you can inform your clients of the daily activities. Here are seven amazing tips that are a must for people interacting on the social media platforms:

1. Make sure that your password is safe

Try to create a unique and unusual password for the social networking site. Make sure that you keep it to yourself only without sharing it with anyone else. In case you doubt that someone might be in possession of your password, then make sure that you change it. An alphanumeric password that does not contain your vital information would be the most secure.

2. Control your privacy settings

You can control the amount of exposure of people to your content by controlling and changing your privacy settings. You can start off by limiting the access to your photos, profile, posts and videos by changing it to ?Friends Only?. The default setting in Facebook, for example, is ?Friends of Friends? that exposes your content to virtually hundreds of people! Make sure that you scroll through the privacy settings of your social networking site and change them appropriately.

3. Be judicious in choosing applications

You will keep getting various requests for applications like surveys, games, quizzes and so on. We end up clicking on the ?Accept? button without realizing that each one of these applications is uniquely devised and some of them gain access to your private information and make it public. Always go through the agreement provided by the application before clicking on the ?Accept? button to avoid all this.

4. Beware of phishing

A lot of dangerous phishing activities are happening via social networking sites. Due to this your account may get infected/hacked by clicking on tempting phrases that appear on your screen. Once your account is infected, the same phishing post will circulate to your friend, causing a chain reaction. For preventing this, make sure you have an Anti-Phishing/Malware application installed on your social networking site apart from being judicious in clicking on tempting messages.

5. Do not accept friends whom you don?t know

Be very judicious when accepting new friends on to your friends? list. You must limit yourself to the number of friends that you really know. Social networking sites only advise the user to accept friends that you have real world dealing with and not people that you vaguely know. Don?t risk yourself and your reputation by accepting any unknown person?s friend request blindly, who might share your information with unknown people and cause a nuisance.

6. Give a thought to your posts

Remember that the posts you make on your social networking site would stick around on the Internet for a while, even if you do remove them later. During that time the videos and photographs might circulate on the web without your knowledge, consequently harming your image in a negative manner. This does not necessarily mean that it is unsafe or unwise to put up posts, but be mindful of the implications that it might have in the future.

7. Be gentle

Avoid using or spreading negativity to other via your posts, comments, photos or videos. Hate messages, violent videos, pornography and so on should be avoided and not encouraged at all. Embarrassing or hurtful content in posts can incite people and hurt them too. Completely avoid spreading any form of negative content that is termed as ?Cyber Bullying?.

The various social networking sites provide a common platform for people to interact from across the various geographies. By using these tips, you can make sure that you are using these social sites optimally and successfully.

Apple iTV Release Coming? TV Set Design Testing Reportedly Under Way At ...

"You know, I used to watch 'The Jetsons' as a kid. I love 'The Jetsons.' We're living 'The Jetsons' with this," Cook said later in that same interview. "It's an area of intense interest. I can't say more than that."

The Apple-made television has been discussed ever since the release of Steve Jobs' biography last November, in which author Walter Isaacson revealed Jobs' intentions to reimagine the television set in the same way his company had revolutionized the personal computer, the music player and the telephone. Shortly before his death in October 2011, Jobs uttered four exciting words to his biographer: "I finally cracked it."

Since then, analysts and Apple fans cannot stop speculating about the Apple-made television set -- when it will be released, how much it will cost, but most importantly, just how revolutionary an experience it will be.

Following Cook's interview with Brian Williams -- the first time he publicly acknowledged that Apple had any interest in the TV experience -- there's been an uptick in news about the rumored "Apple iTV" set.

After a Dec. 12 report from the Wall Street Journal said Apple has been "testing a few designs for a large-screen high-resolution TV" along with Japan's Sharp Corp. and Taiwan's Hon Hai Precision Industry Co. (a.k.a. Foxconn), a new report from Taiwan's Central News Agency says Foxconn is testing those same TV designs this very moment.

"Hon Hai, the world's largest contract electronics maker, has declined to respond to the reports, saying that it will never comment on specific business deals with any single client," wrote Sofia Wu of Focus Taiwan. "However, the Hon Hai source, who spoke on condition of anonymity, said the initial phase of tests on the TVs has kicked off. Nevertheless, the source said it is unlikely that shipments of the appliances will begin as soon as the end of next year."

The Foxconn source said the experimental TV designs require "flat panels ranging between 46 inches and 55 inches, which means that Hon Hai will not need to get its supply exclusively from Sharp," and will probably use panels made by "either Japanese or Taiwanese suppliers."

Jobs' goal with the Apple-branded "iTV" was to reduce the amount of clutter in the TV-watching experience, namely in the number of remotes needed to control the cable channels, as well as peripherals like DVD players and video game systems.?

"I'd like to create an integrated television set that is completely easy to use," Jobs told Isaacson. "It would be seamlessly synched with all of your devices and with iCloud. It will have the simplest user interface you could imagine."

The Apple iTV could potentially use Siri, the company's voice-powered assistant technology, simplify the chore of changing channels, finding shows, setting reminders or scheduling recordings. Some also believe the Apple iTV would feature FaceTime, the company's popular video communication platform, and perhaps third-party apps built specifically for the TV.

Those features are nothing new, however; what would be new would be the rumored "a la carte" experience, which would let TV users buy specific channels they want to watch, rather than own an entire spectrum that may never be used. The interface would tap into individual cable or satellite providers to customize your TV experience around what you want to watch, rather than own thousands of channels that may never be viewed.

Of course, in order for this experience to work, Apple would need to strike deals with all of the major cable and satellite providers, as well as the individual channels. There have been no reports of any major meetings between Tim Cook or anyone at Apple with the TV companies.

Analysts like Gene Munster originally pegged an Apple iTV release in 2012, but considering the processes required to pull off a project like the one currently in the rumor mill, Apple would not likely be able to deliver a branded television set until late 2013 at the very earliest. Two or three years down the line seems far more likely, especially considering that Apple would first need the proper companies to sign on to this project before releasing it to the public in the same way the original iPhone required FCC and carrier approval.

The Apple iTV set may be years away, but IKEA -- yes, that IKEA -- has its own interpretation of an integrated television set available for sale, called "UPPLEVA."

The UPPLEVA is essentially a television set that manages to keep cables, controls, digital boxes and other peripherals embedded directly in the furniture in which each UPPLEVA LED display is housed.?

The TV can play most types of videos, discs and DVDs, but the TV is also Wi-Fi ready and comes with a fair number of Smart TV options, like the ability to search for available programming over different platforms like OnDemand and Netflix. There also are music options, inputs for MP3 players, iPods and even subwoofers, as well as USB slots and HDMI ports for video game consoles and other electronic devices.

The Uppleva is currently available in several European countries, but U.S. customers will have to wait until spring 2013 to buy Ikea's integrated TV/sound/furniture solution. Even though this is Ikea's first major foray into building its own hardware -- and an ambitious piece, at that -- it's doubtful that Ikea will be able to create the same amount of excitement that Apple could with its own branded TV set.

According to Morgan Stanley analyst Katy Huberty, Apple could sell at least 13 million TV set units, with customers willing to pay an average of $1,060, a 20 percent premium over the average $884 they paid for their current TV set.

California Traffic Deaths In 2011 Increased, While U.S. Hits Six-Decade Low

California is bucking nationwide trends when it comes to fatal traffic accidents. While the number of fatal collisions dropped to its lowest point in more than six decades throughout the U.S., California saw an increase in 2011.

According to recently released federal transportation data, 32,367 people were killed in traffic accidents in 2011, a decrease of 1.9 percent from 2010. And while thirty-six states saw a reduction in fatalities, California experienced a 2.6 percent increase, with 2,791 fatal traffic accidents in 2011. That?s more traffic accidents than any other state besides Texas, which had 3,023 fatalities.

Nationally, pedestrian fatalities increased by three percent in 2011, pedalcyclist fatalities increased by 8.7 percent and motorcyclist fatalities increased 14 percent.

A study earlier this year found that in car-addicted Los Angeles, pedestrians accounted for about a third of all traffic fatalities, or nearly triple the national average.

In response, Los Angeles has begun to focus on pedestrian safety. The city?s department of transportation recently hired its first-ever ?pedestrian coordinator,? Margot O?Ca?as.

O?Ca?as told HuffPost in October that she plans to improve signaling, striping, crosswalks, signage and lighting in consideration of everyone from children walking to school to elderly people who have trouble making it across LA?s unusually wide streets.

Mayor Antonio Villaraigosa also announced a new pedestrian safety initiative Monday that will add new visible crosswalks at 53 dangerous intersections.

Traffic safety coalition Watch the Road will also launch an educational campaign, including ads on billboards, bus shelters and bus panels.
